Why my dentist needs to know that I’m “widowed” I do not know. But it’s the first time I’ve had to check that box.

Why, indeed! It is also possible in similar circumstances, to just leave the answer blank and not check any of the options, particularly in places that do not need to know the answer to provide a service. We are so conditioned to answering all the questions. And probably the dentist's office has no idea why some of those questions are there also.
